What is required within a multi disciplinary team is access and freedom to configure the information required for their own service whilst still being able to access and view other services interaction with a patient without the need to move between systems or screens. 
Healthcare professionals need to be able to access their own data in isolation relating to the patient. However total patient care requires that each healthcare professional needs to be aware of other healthcare professionals involvement and the ability to see the detail. A Regional Team can have very complex interactions between different teams and these often change over time. What is required is the ability to set up many different services and within those services individual teams. An example of the type of set up required is shown in the diagram below. Because of the unique way in which iCare has been designed it is possible to define multiple services within one system. Within services individual teams can be defined with the necessary interactions between each team. The key to this approach is that each of these services/teams can be viewed within the patient record. This allows core patient information to be shared amongst all teams saving valuable time and helping keep patient data accurate and any changes available to all. It is possible to have many different instances of the same type of teams. This means there can be, for example, 6 different home care teams each recording their own information separately whilst allowing each to see the other services/teams information (security/access permissions allowing). Once within a patient record the user enters into the Services area and can instantly see all the services that are involved with the patient care as the screenshot below shows. Then by selecting a particular service the user can see the details relevant for this service. 
The Show All button can also be accessed and all of the services can be viewed together. All of the user’s access rights to services and data is controlled by their login security settings. The activity data is also used in the same way enabling users to see their own activity information but also able to view other teams’ interactions with the patient. Core data such as demographics, family, diagnosis, medication etc. is stored once only per patient as all services access the same information. iCare can be used as a regional patient information system allowing multi service, multi team set up and information access. This allows all users access to relevant service information while maintaining the core patient data. This ensures less duplication as well as sharing patient information with all healthcare professionals throughout the region.
